59 years old lady, widow, known case of OA and...
59 years old lady, widow, known case of OA and lumbar disc problem on frequent use of analgesics, she also has dyspepsia with positive urea breath test, currently on eradication regiment. Currently patient develops episodes of localized burning chest pain at the left side with burning at left upper arm and posterior scapular area, followed by burning sensation in abdomen, feeling palpitations, feeling low in everything as patient said, low rate of breathing, fatigue, no shortness of breath, there is sweating in palms, no nausea or vomitingChecked BP is fluctuating; 125/80, 165/90ECG was normalWhat could by her differential diagnosis What kind of work up is needed

She had a history of cardiac surgery done very long time ago, no reports are available, no idea about that surgery, she is saying “it was for closing an opening in her heart”

Wht is the reatment of left sided chest pain and the associated symptoms ?
the treatment is according to the specific cause so she need to visit the internist to get a medical examination and diagnosis of the cause of her condition :
she can take Esomeprazole 40mg for stomache problems but it highly suggestable that the cause is the stomach and the essophagus so she needs to take it parentraly not oraly and to do other investigations
she can take Gemifloxacin 320mg once daily for 5 days for chest infection and if there any other problem like pleural effusion or pulmonary edema she needs admission in the hospital
she can take ibuprofen 600mg for musculoskeletal causes
there are many causes of left sided chest pain that are
chest pain related to cardiac causes such as myocardial infarction or angina
in these cases it's associated with difficulty in breathing, sweeting, shoulder and neck pain
Chest pain related to respiratory causes, such as bronchitis or common cold or lung infection
in these cases, the pain is associated with cough, fever, chest-wheezing
chest pain related to musculoskeletal trauma, in these cause the pain is well localized and for long time and relieved by analgesia
chest pain related to gastroesophageal causes like reflux of stomache acid and hiatus hernia , and indigestion
what tests should she get?
Physical exam
CBC
Electrocardiography ECG
Chest X ray CXR
In some cases she needs to do an ECHO and endoscope
