Dear Customer
Welcome, and thank you for trusting us
Regarding your question
What is the normal change in the thyroid during pregnancy?
The size of the gland increases by 10%. Production of thyroid hormones T3 and T4 increases by about 50%. As a result, the level of normal thyroid stimulating hormone (TSH) during pregnancy is lower than normal TSH level.
The new recommendations for TSH during pregnancy are:
First trimester: less than 2.5
Second trimester: 0.2-3.0
Third trimester of pregnancy: 0.3-3.0.
What does the high TSH level indicate?
If TSH is greater than 2.5 at any time during pregnancy, T4 levels should be checked to determine if hypothyroidism is medically important or medically unimportant
If T4 is low, the diagnosis is a pathologic hypothyroidism which can impair neuronal development of infants. There is also an increased risk of premature birth, low birth weight, and miscarriage. Hypothyroidism should be treated in this case and you should continue on the presccribed medicine
If TSH is high and T4 is normal, the diagnosis is subclinical hypothyroidism . In this case, the next step is to check the peroxidase antibody of thyroid gland. Women who are positive antibodies should be treated as medically important  hypothyroidsm. The effect of subclinical hypothyroidism on the neural development of the fetus is not clear. But one large study showed low IQ tests in children of untreated women.
Treatment is necessary when TSH is 10 or more, regardless of T4 level. In addition, TSH should be monitored every 4 weeks during the first 20 weeks of pregnancy, and then again between 26 and 32 weeks
.
