Can doctor notice if you had sex 4 days of the check time ?
we dont know for sure what do you mean by * notice * , do you mean physically or by test , but the answer is :
- physcially : no , doctor wont notice and wont pay attention to that , because they usually will take history from you and do laboratories tests
- by test : if pregnancy happened already , doctor may notice that although its may be a little early , because pregnancy usually appear on test after 2 weeks of implantation
How to make sure if you are pregnant or not ?
- the best time to do home pregnancy test is first week of next period 
- you can also go see a gynacologist and do a regular check to your productive system and check if there is any early pregnancy changes , which is also in first 2 weeks of next period 
Early pregnancy signs :
1. delayed period

3. frequant urination or urge to urinate
