according to the official website , you have to do the following :
1. first bring your baby Birth certificate
2. second : get Vaccine card issued from the Preventive Health Center for your baby 
what the next step ?
preventive health center are supposed to guide you into the next steps what are more likely to be :
- visit the nearest govermental medical center to your area and get your baby vaccinated 
Vaccination program for newborns :
1. At birth: BCG, Hep B.
2. Two months: DTaP, Hib, Hep B, IPV, PCV.
3. Four months: DPT, Hib, Hep B, OPV, PCV.
4. Six months: DPT, Hib, Hep B, OPV, PCV.
5. 12 months: MMR, varicella.
6. 18 months: DTaP, Hib, OPV, PCV.
7. Five to six years: DPT, OPV, MMR, varicella.

may vaccination center exempt the baby from vaccinations ?
this is not expected , as vaccinations are important to baby health , but you may face the following :
1. doctor may make some changes in the schedule and vaccinations shots 
2. you may have to do another papers work ( we dont know for sure , but in anyway you have to get your baby vaccinated )
expected side effects from vaccination ?
1. mild fever
2. injection area pain and swelling
