Before or during surgery to remove an invasive breast cancer, your doctor removes one or some of the underarm lymph nodes so they can be examined under a microscope for cancer cells. The presence of cancer cells is known as lymph node involvement.
Your results will also tell you how much cancer is in each noderanging from a few tiny cells to many cells that can be seen easily. You might see this reported as:

1.  Microscopic (or minimal): Only a few cancer cells are in the node. A microscope is needed to find them.
2.  Gross (also called significant or macroscopic): There is a lot of cancer in the node. You can see or feel the cancer without a microscope.
3. Extracapsular extension: Cancer has spread outside the wall of the node.

If breast cancer comes back locally, your doctor may recommend some or all of the following treatments, depending on the characteristics of the cancer:

• surgery to remove the affected lymph nodes
• chemotherapy
• radiation therapy
• hormonal therapy if the cancer is hormone-receptor positive
• targeted therapy

 If breast cancer comes back in the lymph nodes in the armpit or collarbone, your doctor may recommend some or all of the following treatments, depending on the characteristics of the cancer:

• surgery to remove the affected lymph nodes
• chemotherapy
• radiation therapy if the area has never been treated with radiation
• hormonal therapy if the cancer is hormone-receptor positive
• targeted therapy

Local Recurrence
A local breast cancer recurrence means that the breast cancer has come back in or near the same place it was originally found in the breast.
Regional Recurrence
A regional breast cancer recurrence means that the breast cancer has come back in the lymph nodes in the armpit or collarbone near the area where the cancer was originally detected
