Causes of back pain:

According to your description, such pain is mostly due to muscle spasm or strain in the upper back and shoulder.
Other possible causes:

• Cervical disc
• Cervical spondyloarthropathy
• Osteoarthritis...etc

Disgnosis of the cause of back pain:

You have to visit an orthopedist for clinical examination and investigations as:

• Cervical and lumbar x-ray
• MRI or CT

Treatment of back pain:

It depends on the cause, and till the right diagnosis, you can use the following treatment:

• Celecoxib 200 mg once daily for 10 days
• Relaxon twice daily
• Diclofene gel twice daily
• Vitamin B complex ( IM injections ) every two days.
