A 59-year-old white woman received an orthotopic liver transplant 6 months ago. She did well postoperatively and was discharged on immunosuppressive therapy. She now presents with a low-grade fever (99.8° F), 5-lb weight gain, tenderness over her transplanted liver, and mild jaundice. She also reports that her urine has become dark and has decreased in amount over the past 2 weeks. Her liver function test results are abnormal. Additional laboratory test results reveal that the serum level of tacrolimus (one of her immunosuppressant medications) is sub therapeutic. Describe the relationship of each of her current symptoms to liver dysfunction. What treatment would you anticipate, and what are the nursing measures for her at this time? What explanation would you give to her for her symptoms?

and according to your discreption it is a Late acute rejection
what is the treatment of acute rejection
- potent immunosuppressant as (tacrolimusand cyclosporin) with appropriate dose and blood level
- corticosteroids
what are the causes
- autoimmunit from the recepient body
- Antibody made on the liver of the donor to attack the recepient cells
what are the associated symptoms
- The organ's function may start to decrease as there will be rellow discoloration of skin and easy bleeding
- General discomfort, uneasiness, or ill feeling
- Pain or swelling in the right upper quadrant of the abdomen at site of liver
- Fever (rare)
- Flu-like symptoms, including chills, body aches, nausea, cough, and shortness of breath
what are the investigation used to diagnose
- clinical exam
- liver function test
- CBC
- abdomen CT scan
