1. Complete sexual intercourse , in which complete penteration occur
2. Penteration using hard subject
3. Trauma to the vagina

? What are the signs and symptoms of Hymen perforation

1. Clinical assessment by a gynecologist is the surest way to diagnose it 
2. Bleeding per vagina
3. Lower abdominal pain
4. Vaginal discharge

So , We can't say that whether it's perforated or not without clinical exam by a gynecologist . If there is no history of full penteration or intercourse , you don't have to worry about it
