32 yr old with mild ly affected valves in RF -has been on Benzathine Penicillin IM every 3 weeks in Yaman.Now in UAE. No H/o reactions before. Does the guideline recommend skin test now for her repeat injection .
Have you seen a doctor about this yet? What medications are you taking?
As written above , on regular Penicillin Injection every 3 weeks .No other medicines

What is the treatment of Rheumatic fever?
1. Antibiotics: like Penecillin,People who have had heart inflammation during rheumatic fever might be advised to take the preventive antibiotic treatment for 10 years or longer.
2. Anti-inflammatory treatment: aspirin or naproxen (Naprosyn), to reduce inflammation, fever and pain.
3. Corticosteroids, if NSAID's didn't help.
What are the complications of RF?
1. Rheumatic heart disease is permanent damage to the heart caused by rheumatic fever. It usually occurs 10 to 20 years after the original illness. Problems are most common with the valve between the two left chambers of the heart (mitral valve), but the other valves can be affected.
2. Valve stenosis. This narrowing of the valve decreases blood flow.
3. Valve regurgitation. This leak in the valve allows blood to flow in the wrong direction.
4. Damage to heart muscle. The inflammation associated with rheumatic fever can weaken the heart muscle, affecting its ability to pump.
Why is the skin test for Penicillin for?
It is to test the sensetivity for Penicillin , if the patient is allergic to it, or shows any signs of hypersensetivity reaction,
Usually it's done once, at the start of the treatment, if the patient is not allergic to Penicillin no further skin testing.
